A chiropractor can address many structural causes of sciatica, particularly disc herniations and vertebral misalignments. Cox flexion-distraction, spinal adjustments, and decompression are all evidence-based options for disc-related cases. Accurate diagnosis comes first. The treatment should match what imaging shows.
True sciatica involves pain that radiates from the lower back into the glute and down one leg, following a pattern based on which nerve root is affected. Pain that stays local to the back without traveling into the leg suggests a different diagnosis. An exam and x-rays are the starting point. MRI provides the full picture when nerve involvement is suspected.
Not necessarily. Many herniated discs reduce in size over time with appropriate treatment. Spinal decompression changes the pressure environment inside the disc, which can help retract the herniation and reduce nerve compression.
Both cause glute and leg pain, but the mechanism differs. Disc herniation sciatica originates at the spine. Piriformis syndrome occurs when the muscle compresses the sciatic nerve in the glute. Treatment differs: decompression for disc-related sciatica, manual therapy and soft tissue work for piriformis. MRI and a thorough exam help distinguish the two.
Most people don’t. Conservative care resolves the majority of cases when applied correctly. Surgery becomes a serious option when there’s severe neurological deficit, when conservative care has failed after 6 to 12 weeks, or when imaging shows structural instability. If a surgical referral becomes appropriate, we have a vetted neurosurgery network in Southwest Florida.
Mild to moderate sciatica from a recent disc herniation: many patients see meaningful improvement within 4 to 6 weeks. Chronic sciatica (6+ months): a full 6 to 10 week treatment course is typical. Piriformis or SI joint-related sciatica: some patients improve in 2 to 4 weeks with manual therapy and shockwave. Progress is tracked and the plan adjusts based on response.
The biggest difference is the MRI requirement before decompression. Most clinics don’t require it. Dr. Bracic has post-graduate training in MRI interpretation and reads every scan himself before building a plan. You know exactly which disc level is being treated and why. The second difference is what’s under one roof: spinal decompression, Class IV AI-guided laser, shockwave, Cox flexion-distraction, manual therapy, chiropractic, and physician oversight.
It can, but one-sided sciatica is far more common. Bilateral sciatica can indicate conditions like cauda equina syndrome, which requires urgent evaluation. If you have pain, weakness, or numbness in both legs, or bladder or bowel changes alongside leg symptoms, seek evaluation promptly.
